CHILDCARE ARRANGEMENT FORMS

**NEW PROCEDURE**

Starting with the 2017-18 school year, Childcare Arrangement Forms must be updated ANNUALLY. You will need to complete a new form if you have a childcare provider. Previous years forms will not be recognized. Childcare Arrangement Forms can be picked up from the Transportation Department, Central Office, or fax the Childcare Arrangment Form to 484-7073.
